Person Centred Assessment and Support Planning

This course is intended for all staff that have a role in the assessment and support planning process for the individuals that they work with.

It covers the key principles of assessment and support planning (i.e. assessment, goal setting support planning, implementation and review) and focuses on how these can be achieved in a person centred way.
This can be delivered as a one or two day course: the two day course will enable learners opportunities for practice in setting goals and translating these into plans, along with exploring how these link to risk assessments.
Learning Outcomes
By the end of this course learners should be able to understand:
- The support planning process (assessment, goal setting, planning, implementation and review)
- How this can be achieved in a person centred way
- How they can ensure that service users are involved in the process
- How to manage situations where an individual does not wish to be or cannot be involved in the process
- The importance of SMART goals/outcomes
- What is meant by need (and the difference between need and want)
- The importance of involving key stakeholders in the process
- The importance of ensuring that plans are consistently followed and kept ‘live’
